Question: what is a lipid evaluation charge on my cardiac med labs?

I had my usual pre cardiologist visit labs. He had asked for three items.
1) lipid evaluation
2) AST SGOT
3) ALT SGPT

When the charges came in there were four charges. Pretty nebulously annotated as usual:
OUTPT LAB $22
OUTPT LAB $91
OUTPT LAB $21
OUTPT LAB $21
The lab says the $91 charge was for the “lipid evaluation” by a pathologist. My question is; What is there to evaluate? Just do the lab work, post the results, and the cardiologist or anyone with a web browser can “evaluate” the results. There are plenty of published guidelines for Total Cholesterol, Triglycerides, HDL, LDL, and CHOL/HDL ratio. I pay for these labs out of my pocket since my deductible is so high and I don’t need a spurious charges. I feel like I am being robbed.

Yes, these charges are rather silly in my opinion. I am a clinical lab scientist and we are actually the ones that review the labs before releasing them to the physician. It’s not like we can charge a patient for this. There is no way that a pathologist reviews all of the lab results for every patient. I would ask them if it was evaluated then you would like a copy of the pathologist intepretation for every single one of the charges and see what they have to say.

Answer: Best to get yourself an accountant to do that since the local tax code is so “negotiable”. Ask any BIR agent about it, they will tell you it’s “negotiable”, meaning anything is acceptable with the right amount of grease money. You can study form, I think it’s 1702, the blanks on there will be specified on what the deductions will be. From what I remember it’s basically what will be in your income statement so it’s Cost of Goods Sold which is basically your Purchases, and Operating Expenses which include taxes and Licenses, Salaries and Wages, SSS Medicare, and EC, Utilities, Telecom, Security (as in guards), Fuel, Rental, and some Misc expenses. Question: Is my mileage deductible?

I’ve worked a promotions job for the past two and a half years but 2009 will be the first year I am itemizing my taxes. I drive a lot for this promotions job and we are reimbursed for mileage over 30 miles from home. So if I drive 130 miles, I get reimbusred for 100. If I drive 25, I get reimbursed for nothing. My question is am I able to deduct the 30 miles from my taxes? Am I able to deduct the mileage driven under 30 miles? I haven’t kept all of my records for the year so I would only be deducting for those I have timesheet and expense sheet information for. If I am able to deduct these, I am going to keep a detailed mileage log in the future. Thanks for any help!
I forgot to mention that I do have a full-time job I work as well, so that is not my only job.




Answer: The first and last trips of the day are commuting miles and are not deductible. All other miles are deductible.

You can take the standard mileage rate (currently 55¢ per mile) for all business miles or the actual cost rate, apportioned for the business use. Either way you need to keep a detailed record of all business miles For record keeping purposes the standard mileage rate is much easier. You must subtract your employer reimbursements unless they are included in Box 1 of your Form W-2.
